VETERINARY REFERRAL AND CONSENT
In accordance with the Veterinary Surgeons Act 1966, it is a legal requirement to obtain veterinary consent prior to the treatment of an injured animal or an animal that has a diagnosed condition*.
*Patients do not require veterinary referral if treatments are for maintenance or fitness purposes only.
This is quick and easy to do. You can either download my veterinary referral form from the links below and give this to your vet to sign either in person or via email, or alternatively you can call your vet and verbally ask for their permission and sign the declaration form confirming you have received consent.
